A single abnormal report isn't a diagnosis: sperm numbers swing with illness, fever, stress and abstinence period, and sperm take about 2–3 months to develop, so a repeat test after several weeks is standard. For reference, the WHO (2021) lower reference limit for concentration is 16 million/mL. A urologist/andrologist can look for treatable causes — a varicocele, infection, hormone imbalance, heat exposure, tobacco, alcohol or certain medicines. Even with severe male factor, ICSI (injecting a single sperm into each egg) lets many couples conceive with IVF. Reassure each other that this is a medical finding, not a failing.
