What is a sperm DNA fragmentation test and do we really need one?
My husband's semen analysis came back normal but we've had two failed IUIs. Someone suggested a DNA fragmentation test. Is it useful or just an extra expense?

A sperm DNA fragmentation (SDF) test looks at how much of the genetic material inside sperm is damaged or broken. A standard semen analysis checks count, movement and shape, but it cannot see DNA quality, so a man can have a normal report and still have high fragmentation. The test is not routine for every couple. Guidelines differ on how useful it is, because results vary between labs and methods and it does not always change what treatment is offered. It is more commonly considered in situations like repeated pregnancy loss, repeated IUI or IVF failure without a clear reason, a varicocele with borderline semen, or known risk factors such as smoking, obesity, high fevers or exposure to heat or toxins. If fragmentation is high, doctors may first look for treatable causes: stopping smoking, reducing alcohol, managing weight, treating an infection or varicocele, and avoiding heat. Shorter abstinence before giving a sample can sometimes help too. In some cases a different lab technique or treatment approach may be discussed. Ask your doctor whether the result would actually change your plan before doing it. If it would, it can be worth it; if not, the money may be better spent elsewhere. Your fertility specialist or an andrologist is the right person to interpret the number in context.
