Long or unpredictable cycles often mean ovulation is infrequent or late — but not necessarily absent. Common causes include PCOS, thyroid problems, raised prolactin, significant weight change, heavy exercise, stress, and (in the late 30s and 40s) the approach of perimenopause. A gynaecologist will usually check a pelvic ultrasound, TSH (thyroid), prolactin, and sometimes androgens and AMH. If progesterone is tested to confirm ovulation, timing matters: it should be drawn about a week before your *expected* period — the old 'day 21' rule only suits a 28-day cycle. Irregular ovulation is one of the most treatable causes of delay, so it's worth getting checked rather than waiting.
